- Photocopies of Secondary School Certificate(s)
- Other certificates or diplomas if applicable
- Two recent passport size photographs signed by the applicant (attach one on the form)
- A letter of recommendation from the former school
- A receipt of payment of the application fee of UGX 50,000 paid at the University Bank Account
